• Initial and follow-up assessment sessions
• Comprehensive Functional Capacity Assessment report
• Assistance with completing the NDIS Access Request Form
• Accompaniment to the GP appointment for completion of the NDIS Access Request Form

Recommended for clients applying for NDIS funding.

If you have a GP referral under a Chronic Disease Management Plan, you may be eligible for a Medicare rebate of $61.80, reducing your gap fee to $38.20.

Please bring your referral form to your first appointment.

Sessions are tailored to individual goals, focusing on daily living skills, independence, and emotional regulation. 

Supporting clients who are plan or self-managed through NDIS.

Reports and professional letters (e.g. for medical purposes) are billed according to the time required at our standard rate of $193.99 per hour.

A quote will be provided once your needs and report requirements are discussed.
