At OT Services & Assessments, we believe in transparent, ethical, and accessible pricing. Below is an overview of our standard service fees. All costs are inclusive of travel within the local area and standard administrative time.
1. Functional Capacity Assessment
$2000
Includes:
- Initial and follow-up assessment sessions
- Comprehensive Functional Capacity Assessment report
- Assistance with completing the NDIS Access Request Form
- Accompaniment to the GP appointment for completion of the NDIS Access Request Form
Recommended for clients applying for NDIS funding.
2. Medicare Chronic Disease Management Plan
$100/session
If you have a GP referral under a Chronic Disease Management Plan, you may be eligible for a Medicare rebate of $61.80, reducing your gap fee to $38.20.
Please bring your referral form to your first appointment.
3. Private OT Sessions
$100/hr
Sessions are tailored to individual goals, focusing on daily living skills, independence, and emotional regulation.
4. Reports and Supporting Letters
$100/hr
Reports and professional letters (e.g. for medical purposes) are billed according to the time required at our standard rate of $150 per hour.
A quote will be provided once your needs and report requirements are discussed.
5. Form Completion
$100/hr
Pricing varies depending on the type and complexity of the form.
Forms may include (but are not limited to):
- Disability or Mobility Parking Permits
- Companion Card Applications
- Department of Housing Support Forms
- Centerlink Medical Reports

Financial Hardship Consideration: We understand that circumstances vary. Financial hardship will be considered on a case-by-case basis, and flexible payment plans or reduced pricing can be arranged upon request.
